If you’re having trouble submitting and sending in the information forms for the Ketogenic Meal plan program, this typically only happens when the “check boxes” are not checked. On a tablet or phone with a small screen, these checkboxes can easily be overlooked sometimes. especially the 1 up top

for the dog information form there is a single check box at the very top/beginning of the form and then again at the very bottom of the form you will see 5 more check boxes. If ALL 6 of these boxes are not checked, your form will not send.

And for the food sourcing form, there is 1 check box at the very top of screen and 1 more checkbox at the bottom of screen. both must be checked in order for form to send
